Attendee Personal Release

The Design and Management of Cancer Clinical Trials Course is a recorded event. By attending this event, you consent to your voice, image, and likeness being recorded for use and dissemination, without payment of any kind for such use, in any language, format, or media now known or later devised, and you release the University of Miami from any and all claims, liability, or damages arising from any such use. The University of Miami will proceed in reliance upon such consent and release. If you do not consent to the foregoing, please do not attend the event.

Attendee Surveys

Participant feedback is important for continuing education accreditation and for continuous improvement of this course. We will distribute surveys to all participants after each weekly session to collect input on course content and format, and a few months after the course ends to assess any practice changes as a result of the course. We kindly request that you complete surveys for all sessions that you attend.
